RCCHC Provides Integrated Care

Integrated Care combines primary health care and mental health care in one setting. Providing Integrated Care helps patients and their providers by blending the expertise of mental health, substance use, and primary care clinicians, with feedback from patients and their caregivers. This creates a team-based approach where mental health care and general medical care are offered in the same setting.

RCCHC Monthly Wellness Group

RCCHC hosts a Wellness Group, open to all patients, on the second Wednesday of every month, from 10-11am. Group meetings are held in the boardroom on the second floor of Ahoskie Comprehensive Care, located at 120 Health Center Dr. Ahoskie, NC 27910.

Questionnaires are NOT intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the guidance of your doctor or other qualified health professional with any questions you may have regarding your health or a medical condition.

  • 1. For each question, circle the number that best matches your mood or thinking for the past two weeks.
  • 2. Add up each column and write the total on the corresponding line underneath question 9.
  • 3. Add together the total for each column for your final score.
  • 4. A score of 9 or greater may indicate a need to see a Behavioral Health provider.

Regardless of your PHQ-9 or GAD-7 score, if you feel you would like to see a Behavioral Health provider, you are welcome to schedule an appointment by calling any RCCHC location.
